Summary

  • Position Summary
    • The Social Worker for works as part of the interdisciplinary team to provide individuals, families, and care givers with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising patients and family care givers of community resources and supports, helping to guide individuals through the process as needed.  The social worker will provide education where appropriate, empowering patients to share in the decision making process when determining their options for treatment and/or need to access services appropriate to meet their individual health needs. The Social Worker may also provide care and case management or interventions designed to promote health, prevent disease and address barriers in access to healthcare. 
  • Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: Bachelor’s degree in Social Work.
    • License/Certifications: Current Maine licensure as a LSW.
    • Experience: Office setting experience.
